Open Records Requests

The University of Louisville Foundation complies with the Kentucky Open Records Act (KRS 61.870-61.884). Residents of the Commonwealth of Kentucky seeking access to records covered under this statute must apply in writing.

A letter identifying the records sought may be submitted by mail, fax or email to:

The request should reference the Kentucky Open Records Act, and include the following:

  • requestor’s name;
  • mailing address;
  • description of the records requested;
  • a statement regarding whether the request is for a commercial purpose;
  • a statement that the person making the request is a resident of the Commonwealth under KRS 61.870 (10), including stating the manner in which the requestor is a resident of the Commonwealth under KRS 61.870 (10)(a) to (f); and
  • signature of the requesting party.

A completed, standardized form available on the Kentucky Attorney General’s website may be submitted to request records.
